Patient Surveys

Would you be willing to respond to occasional surveys about the practice, the services we provide and in the future the services we commission on your behalf?

If so please email the practice at sshc@mkpct.nhs.uk confirming your willingness to participate in occasional surveys.  Each survey should take no more than 5 minutes to complete and we would expect to send up to 10 surveys per year. 

Surveys will be sent out to your registered email address for as long as you are happy to participate.  You can choose to opt out at any time. 

The surveys will commence once we have sufficient patients who have registered an interest.

Telephone Sales Scam

We have been made aware that an organisation is calling patients claiming to be from the health centre and offering a cure-all drink for sale.

 

Please be aware this is not supported by the practice, has nothing to do with the practice and we have informed the Police and Trading Standards who are acting on the information given.

 

Please do not fall victim to these hard sales techniques.  If in doubt ring off and telephone the practice for clarity.  Dec 2010

 

 

How do I get an appointment?

Please note:

You can book appointments in advance. 

You do not have to call on the day to book a routine appointment. 

Please ask at reception for the first available appointment. 

Alternatively please ask for a telephone call. 

Please do not ring off frustrated! 

We will offer you something appropriate for your needs.

Please do make a note of who you are speaking to when you ring the practice (the receptionist will say her name when answering the telephone) and if you have any comments please contact the Assistant practice manager, Gill Hulf.

 

 

Online, Repeat Medications Update

In response to a patient request our website developers have increased the number of possible, regular repeat medications, (for use when ordering repeat medication online) from 20 to 30 items.  We trust you find this useful.  June 2010

Repeat Prescriptions

pillsIf you are on regular medication, and after consultation with your Doctor, repeat medication may be requested. This can be by personal attendance at reception during opening hours, by letter, by fax on 01908 560913, or by following the link on this page to the online form.

Please note that for safety reasons we are unable to take telephone requests for repeat medication.

Please allow 2 working days before collecting the prescription from the health centre, or 3 days if you prefer to nominate a local pharmacy collect the dispensed medication from. Some of the pharmacies will provide a home delivery service for housebound and elderly patients.
