Watling Street Primary Care Networks integrated community support team (ICST) are deployed within the PCN to support our more vulnerable patients to remain in their homes and out of hospital for as long as it is safe for them to do so.
The team is able to draw upon the skills of a Specialist Nurse, a Cancer Care Nurse, a Social Prescriber, an Occupational Therapist, a Mental Health Practitioner, a Health and Wellbeing Coach and Health Care Assistant. The diversity of skills and breadth of experience across this team leaves them well placed to support our patients.
If your GP believes you would benefit from assistance from the ICST our team will be in contact with you to arrange an appointment to review your needs. Often this appointment can be done in your own home, which is especially beneficial if you have just had a spell in hospital.
